Position Overview People Care is seeking a bilingual Chinese/English Home Care Coordinator to provide home care coordination support for clients served through the DFTA / NYC Aging programs . This position is primarily responsible for managing assigned client service cases and ensuring that clients receive stable, continuous, and uninterrupted home care services. The successful candidate must be able to communicate effectively in both Chinese and English with clients, family members, and Home Health Aides (HHAs), while serving as a key point of coordination between clients, caregivers, and internal teams. This is a full-time, on-site position at our Brooklyn office. Remote or hybrid work arrangements are not available. Key Responsibilities Manage assigned DFTA client service cases and ensure that clients' care needs are met. Ensure that all clients with approved services are assigned appropriate caregivers and that all shifts are covered. Match clients with appropriate Home Health Aides (HHAs) based on language needs, geographic location, work schedules, and individual preferences. Accurately maintain scheduling information and Electronic Visit Verification (EVV) records in HHAeXchange. Follow up daily and resolve missed visit records and EVV-related issues. Respond promptly to caregiver call-outs, schedule changes, and staffing gaps. Address caregiver No-Call/No-Show situations in accordance with company procedures. Maintain accurate authorization documentation, client records, and related files. Confirm the following day's schedules and promptly fill any open shifts. Maintain timely and effective communication with clients, family members, and caregivers. Work closely with the Clinical, Compliance, Billing, Intake, and Coordination teams. Support operational optimization and continuous improvement initiatives. Required Qualifications At least one year of experience in home care coordination, caregiver scheduling, or a related role. Experience using HHAeXchange or a similar scheduling/EVV system. Fluency in both Chinese and English. Ability to communicate effectively in Chinese with clients, family members, and caregivers. Ability to use English for work documentation, recordkeeping, and daily communication. Strong organizational, coordination, communication, and problem-solving skills. Ability to manage multiple client cases simultaneously in a fast-paced work environment while maintaining strong attention to detail. Preferred Qualifications Experience working with DFTA / NYC Aging programs. Experience with NHTD, CDPAP, MLTC, or other government-funded and managed care programs. Ability to read and write in Chinese. Familiarity with the Sunset Park and Bensonhurst communities. Why Join People Care?
