This work depends on being in the room with the coordination team. Required Bilingual English and Spanish People Care has served New York patients and families for 50 years. We employ roughly 4,300 caregivers across New York City and Long Island. This seat owns a NHTD and managed care case list — you decide who is staffed, what gets covered, and what does not fall through. WHAT YOU OWN Your case list, end to end. Every authorized client staffed, every shift covered, continuity of care held. This is the one outcome the seat is measured on. Aide-to-client matching. Matched on language, geography, schedule and client preference — and kept stable over time, because continuity is the whole point. Scheduling and EVV integrity. Accurate records in HHAeXchange, with the exception queue worked daily so missed and unverified visits are resolved same-day. Coverage under pressure. Call-outs, schedule changes and gaps managed without a break in service. No-call/no-show escalated immediately per the hold process. Program documentation. Authorizations, case notes and records kept current and audit-ready against NHTD and managed care requirements. WHO YOU ARE You are someone who cares deeply about older adults and the aides who serve them. You are transparent — you surface a coverage problem early rather than hoping it resolves itself. You go beyond what the task asks when a client's care depends on it. You row together with clinical, compliance and billing rather than working around them. 1+ year or more of home care coordination or scheduling experience. HHAeXchange fluency , or a comparable scheduling / EVV system. A full case list, held without losing detail. You prioritize under pressure and nothing quietly slips. Clear, calm communication with clients , families and aides — including the difficult conversations. Comfort in a structured operating system (we run on EOS). NICE TO HAVE Experience with NHTD and managed care, or a waiver or managed-care program (NHTD, CDPAP, MLTC). Bilingual in any language spoken by our client population.
