Full-Time Licensed Physical Therapist - Skilled Nursing Facility

Work Environment In this onsite role, you will work directly within the skilled nursing facility, which fosters an atmosphere of teamwork and collaboration among healthcare providers. Our facility is equipped with the latest rehabilitation technologies and resources, allowing you to implement evidence-based practices effectively. You will work Monday through Friday, from 9:00 AM to 5:00 PM, allowing a respectful work-life balance while still providing exceptional care to our patients. Key Responsibilities Your daily activities will encompass a range of responsibilities, including: Administering individualized therapy programs tailored to each patient's needs, focusing on strength and mobility recovery. Conducting comprehensive evaluations of patient conditions and progress to inform treatment plans. Collaborating with multidisciplinary teams, including nurses, occupational therapists, and rehabilitation aides, to ensure coordinated care. Communicating effectively with patients' families to provide updates on progress and address any concerns. Maintaining meticulous documentation in compliance with regulatory standards to ensure continuity of care. Utilizing specialized equipment and tools in therapy sessions, adapting techniques based on individual patient capabilities. Patient Population / Client Focus You will primarily work with a diverse patient population including elderly individuals recovering from surgeries, strokes, or falls, as well as those living with chronic conditions requiring ongoing rehabilitation. Your role will significantly influence their recovery and rehabilitation process, contributing to improved functional independence and overall well-being. Qualifications Possession of a valid and active New York State Physical Therapist License. A Bachelor's degree in Physical Therapy is mandatory; however, candidates with a Master's or Doctorate degree are preferred. At least two years of hands-on experience in a skilled nursing or similar healthcare setting is required. Preferred Experience While two years of experience is required, candidates with experience in geriatric rehabilitation or those familiar with home care settings will be given preference. Familiarity with advanced therapeutic techniques, including manual therapy and neuromuscular re-education, will also be considered valuable. Compensation and Benefits This position offers an annual salary ranging from $100,000 to $120,000 based on experience, accompanied by a comprehensive benefits package.
