Hospice Registered Nurse Case Manager

Optum is seeking an experienced Registered Nurse Case Manager for hospice home visits in New Jersey. You will assess patients, develop and implement plans of care, and collaborate with an interdisciplinary group to deliver compassionate care. A valid RN license and reliable transportation are required. Full-time position with standard business hours; on-call rotations may apply.
