Embark on a career that fulfills more than just professional goals—it transforms lives every day.About This Role:Care Coordination & Patient Engagement: Accepts warm hand-offs from medical and behavioral health providers to ensure seamless integration of care; engages patients (adults and children) and families in setting and achieving self-care goals through individualized care planning and support.Health Education & Coaching: Delivers in-person and virtual health education sessions—both individual and group—using evidence-based curricula to support patients' self-management and wellness.Case Management & Support Services: Provides case management, tracks patient progress, coordinates referrals to community resources, and monitors ongoing care plans to ensure medical, behavioral, and psychosocial needs are met.Scheduling & Access to Services: Coordinates and schedules appointments across multiple service lines, including medical, dental, nutrition, and behavioral health; educates patients on available Evara Health services and facilitates access.Reporting & Committee Participation: Collects and reports program data, presents patient case reviews, participates in CMHI Oversight Committee, and supports special projects and organizational compliance with HIPAA, HRSA, and other regulatory standards.Why You'll Love Working Here:Impact: Every day, you'll make a significant impact on our patients' lives, leading efforts that go beyond healthcare to ensure community wellbeing.Growth: We support your professional development through continuous learning and opportunities to grow within Evara Health.Recognition: As part of our team, your hard work will be recognized and rewarded, contributing to your professional fulfillment and job satisfaction.Education and Experience:Bachelor's Degree in Social Work, Health or Human Services field, preferredMinimum of three years' experience with minimum qualifications listed aboveCulture and Benefits:What sets Evara Health apart is our amazing culture and team spirit.
