This position works with individuals experiencing homelessness in a shelter setting and is responsible for supporting their transition from shelter to stable, permanent housing. The Case Manager collaborates closely with community partners to coordinate services and participates in joint efforts to help participants complete housing assessments, enroll in the Oregon Health Plan (OHP), obtain documentation required for housing and benefits, and connect to local resources. The role also includes attending community meetings, contributing to coordinated community responses, and participating in outreach efforts as needed. The Case Manager may focus on one or more service areas, including case management for young adults ages 18–24, case management for singles, couples, and families, and/or community health work that integrates health and housing supports.

Hours: Monday - Friday 8am - 4:30pm (occasional weekend and evening hours)

Wage: 25.76: DOE + DOQ

Preferred Qualifications:

• Associate degree in human services, social work, or sociology preferred.
• Two (2) years of case management or social services experience.
• Demonstrated experience in mediation between persons exhibiting escalated behavior.
• Strong ability to work collaboratively with other case managers, healthcare providers, and social service agencies.
• Experience working with individuals experiencing homelessness, chronic illness, behavioral health challenges, and/or substance use disorders preferred.
• Strong organizational skills and ability to manage multiple participants' needs effectively.

Other Requirements:

• MUST have a valid Oregon Driver's license; must have been a licensed driver for at least three (3) years; must have a driving record that permits coverage under View email address on click.appcast.io's company insurance.
• All applicants must successfully pass a preemployment drug screening prior to beginning employment.

Essential Job Duties Include (But Not Limited To):

Data, Documentation, and Compliance

• Maintains thorough, accurate, and timely documentation of participant interactions, services provided, and progress toward goals.
• Submits required data, case notes, and reports in accordance with program, organizational, and grant requirements.
• Ensures all protected health information (PHI) is handled in compliance with HIPAA and organizational confidentiality standards.
• Participates in training and professional development initiatives to strengthen team capacity.
• Coordinates medication pickup and delivery for participants as needed, ensuring secure handling, accurate completion of receipts/forms, and adherence to established View email address on click.appcast.io procedures.

Build Relationships and Trust with Participants

• Supports participants in taking next steps toward stability by connecting them to resources, including OHP, domestic violence services, substance use treatment, and behavioral/mental health services.
• Conducts regular participant check-ins from program entry through ongoing support, addressing needs that may impact shelter and housing retention.
• Assists participants in pursuing personal goals, communicates progress with the Site Manager and Case Management/Peer team as appropriate, and helps address barriers while honoring participant dignity, choice, and self-determination.
• Provides individual and group education on life skills (e.g., appointment management, nutrition, hygiene, and navigating systems independently).

Creating Access to Shelter and Housing Opportunities

• Works alongside participants to identify and address barriers to shelter and housing opportunities.
• Assists participants by providing housing navigation support.
• Supports participants through transitions with services or programs — internally or externally—to ensure collaboration and continuity of care.

Cultivating the Mission and Values of View email address on click.appcast.io Through Community Partnerships and Collaboration

• Demonstrates commitment to trauma-informed care and harm-reduction practices.
• Network with and attend meetings alongside social service agencies to promote interagency communication, coordination, and continuity of care.
• Participates in collaborative efforts with partner organizations (e.g., Salem Housing Authority, HIV Alliance, Easterseals, ARCHES, Northwest Human Services, etc.).
• Collaborates respectfully with all team members, aligning with View email address on click.appcast.io values—seeing with compassion, building relationships, practicing empathetic listening, and pursuing peacemaking—while applying an outward mindset and Nonviolent Communication principles.
• Attends required trainings and team meetings and maintains working knowledge of View email address on click.appcast.io operational policies and procedures.

Work Environment

• Work is performed in close quarters with vulnerable populations, including individuals without stable housing who may be experiencing mental health and/or substance use challenges.
• Indoor/outdoor work environment with frequent interruptions, competing demands, and occasional unpredictable behavior.
• Travel between Church at the Park–managed sites, community partner agencies, and encampments throughout the area.

Supervision & Guidance:

• This position operates under general supervision and will receive regular direction, support, and performance feedback from leadership.
• The role requires initiative, reliability, and regular communication with supervisors regarding project progress, challenges, and development needs.

Physical Requirements:

• This position requires walking long distances outdoors in adverse weather conditions.
• Must be able to go up and down stairs.
• Must be able to push, pull, and/or lift 50 pounds or less.
