Medical Social Worker

We are looking for an experienced, talented, and compassionate Hospice Social Worker for a part‑time 24‑hour position at the Pat Roche Hospice House. What will you be doing: The Hospice Social Worker will work as part of a home‑based interdisciplinary team, providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving palliative or hospice services through Croí Health. You will conduct an initial psychosocial assessment of the patient/caregiver, contributing to the agency’s plan of care, and strive for optimal patient outcomes with improved functioning and an increased ability to self‑manage their bio‑psych‑social challenges. You will provide medical services to hospice patients and/or family members reflective of the philosophy, mission, and standards established by the agency. Compensation: $38/hr – $52/hr based on experience and license.
