Responsibilities Support individuals in Group Homes with their daily activities, including personal care. Work closely with individuals to help them achieve personal goals that allow them to live as independently as possible. Provide supports as specified in the Life Plan and Staff Action Plan for individuals in Community Habilitation settings.
