Job Details Job Location: Latham, NY 12110 Position Type: Per Diem Salary Range: $18.45 - $18.45 Hourly Community Habilitation – Direct Support Professional This position is less than 20 hours per week Pay rate $18.45/hr Requirements High school diploma or general education degree (GED). A valid NYS driver's license and ability to drive, with a driving record acceptable for agency insurance coverage. Access to a vehicle needed to provide services. Who are we looking for? Summary As a community Habilitation DSP you are matched to an individual based on likes, location and availability. Generally you will be going out into the community with the individual. This position requires a great amount of commitment, self‑motivation and solid communication skills. Essential Duties and Responsibilities The work is usually one‑to‑one. Participates in a person‑centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities. Assists individuals to identify social, recreation and other community resources they may wish to access and devises plans to achieve that access. Negotiates schedules and daily activities directly with the individual and/or caregiver; establishes and maintains regular hours with individual(s) that you work with. Completes all mandated reporting requirements by Wildwood and regulating agencies. Other duties may be assigned.
