Part-Time Home Health Physical Therapist

Role Overview: A Home Health Physical Therapist provides personalized in-home physical therapy to help patients improve strength, mobility, and independence. Reporting to the Clinical Manager, responsibilities include planning and administering treatments, assessing patient strengths, developing rehabilitation programs aligned with physician plans, supervising PTAs and aides, and documenting progress. Assess and document patient physical status to guide treatment goals. Implement therapy plans to optimize functional recovery. Oversee care provided by support staff, ensuring clear instructions and proper documentation. Communicate patient progress to the care team, physicians, patients, and families. Qualifications & Skills Current, unrestricted Physical Therapy license. CPR certification. Experience working with healthcare teams. Preferred: One year of physical therapy experience. Additional Details: Requires weekends (Friday-Monday), a valid driver’s license, vehicle insurance, and TB screening.
