Home-Based Physical Therapist
Join the In Home Rehab Group team as a Home-Based Physical Therapist and make a meaningful difference in the lives of our patients!
As a Home-Based Physical Therapist with In Home Rehab Group, you will have the unique opportunity to provide high-quality physical therapy services to patients in the comfort of their own homes. Our outpatient physical therapy and occupational therapy services are designed to cater to the needs of our patients, and we are seeking a dedicated and compassionate professional to join our team.
Responsibilities:
Conduct physical therapy evaluations and treatments in patients' homes, utilizing a variety of evidence-based techniques and interventions
Develop and implement individualized treatment plans to address patients' goals and needs
Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care
Maintain accurate and timely records of patient progress and treatment plans
Participate in professional development opportunities to stay current with best practices in physical therapy
