What You'll Do

• Manage and coordinate care for a patient caseload in the community.
• Perform comprehensive nursing assessments and develop individualized care plans.
• Collaborate with physicians, social workers, therapists, chaplains, and other healthcare professionals.
• Educate patients and families regarding treatment plans, symptom management, and available resources.
• Advocate for patients while ensuring continuity of care and positive clinical outcomes.
• Build trusted relationships with patients and families during some of life's most important moments.
• Document care accurately and maintain compliance with clinical and regulatory standards.

What We're Looking For

• Active California RN license.
• At least one year of nursing experience.
• Experience in Case Management, Home Health, Hospice, Acute Care, Care Coordination, Skilled Nursing, Utilization Management, or related nursing environments is highly valued.
• Strong assessment, communication, and relationship-building skills.
• Ability to work independently while collaborating within a multidisciplinary team.
• Valid driver's license and reliable transportation.
