Registered Nurse (RN) - Case Management

Case Manager – Acute Care

Schedule: Day shift Every other weekend required

Assignment Details:

• Contract Length: 26 weeks
• Guaranteed Hours: 25 per week

Requirements:

• Valid state license required; pending accepted if active by start date
• Minimum of 2 years acute care case management/discharge planning experience
• Travel experience required; first-time travelers not accepted
• Proficient in Epic

Roles & Responsibilities:

• Assess patients and coordinate care plans
• Manage discharge planning and utilization review
• Facilitate family meetings and contribute to treatment planning
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and transport
• Collaborate with interdisciplinary teams, including Physical Therapy and Social Services

Compliance / Notes:

• Orientation: 40 hours
• One request for time off allowed, must request upfront
• Commitment to working 3 of 5 specified holidays required
