Waymark is seeking an Outreach Specialist/Care Coordinator in the Chicago area. You will be the frontline liaison with patients at healthcare facilities, building trusted relationships and guiding them to resources with a multidisciplinary care team. Responsibilities include outreach, data review from EHRs, and coordinating with primary care providers.
