Key Responsibilities

As a Physical Therapist at Break Through Rehab, your day-to-day responsibilities will include:

• Thoroughly reviewing patients' medical histories to understand their physical limitations and health conditions.
• Engaging in consultations with patients to assess their physical conditions and symptoms, fostering a trusted therapeutic relationship.
• Diagnosing movement dysfunction and creating personalized treatment plans that outline specific goals and interventions for each patient.
• Educating patients on the proper techniques for therapeutic exercises, ensuring safety and effectiveness.
• Administering physical interventions such as massage, stretching, and manual therapy to promote healing and improve mobility.
• Guiding patients on the safe use of rehabilitation equipment, including wheelchairs, walkers, and other assistive devices.
• Meticulously maintaining accurate patient records including progress notes, treatment plans, and outcome assessments.
• Providing comprehensive guidance to patients and their families on in-home treatment options and exercises for continued recovery.
• Collaborating with other healthcare professionals to ensure a holistic approach to patient care, sharing vital information for comprehensive evaluations and treatments.

Qualifications

To be considered for this role, candidates must meet the following qualifications:

• A strong background in assessing and treating a variety of health conditions, along with proficiency in utilizing electronic health record systems for documentation.
