Registered Nurse - Hospice Care

Schedule

Monday to Friday from 8 AM to 4 PM, with on-call and weekend rotation required, including holidays.

Job Description

The Registered Nurse will provide quality care to assigned patients utilizing the nursing process in compliance with evidence-based practice, State and Federal regulations, and relevant nursing policies. This role includes managing patient care plans, performing OASIS documentation, and ensuring effective communication among interdisciplinary teams.

Responsibilities

• Assess patient/care giver needs and develop a realistic plan of care.
• Serve as Case Manager for assigned patients, managing case loads effectively.
• Accurately complete OASIS documentation in compliance with agency policies.
• Implement medical plans of care and nursing interventions.
• Stay up-to-date on high-tech skills and infection control standards.
• Work collaboratively with LPNs, delegating tasks appropriately.
• Evaluate the effectiveness of plans of care and adjust as necessary.
• Maintain accurate documentation of nursing care provided.
• Provide patient and care giver education on medication, treatment options, and services.
• Coordinate interdisciplinary needs for patients, ensuring timely communication with other disciplines.
• Complete necessary agency and federal forms during visits.
• Assess patient needs for HCA services and coordinate plans of care.
• Transmit patient incident reports to QI Specialists promptly.
• Coordinate safe patient discharges and transfers.
• Communicate effectively with physicians, providers, and team members regarding patient care.
• Participate in peer review audits, providing and accepting constructive feedback.
• Facilitate case conferences to maximize team communication.
• Advocate for patient care issues, ensuring sensitivity to the specific needs of patients and families.

Required Experience / Certifications / Licensure

• Current RN license in the state of practice.
• OASIS experience is required.
• Prior experience in hospice care is preferred.
