About This Role The Field Case Manager RN will work with patients primarily in their home setting as part of an interdisciplinary care team. This role is ideal for those who are passionate about providing holistic care and navigating patients through the healthcare system. Highlights Pay: $58,800 - $105,000 annually Location: Grove City, OH Facility: Optum Home & Community Care Brand/Division: Optum Schedule: Full Time Sign-On Bonus: $2,500 for External Candidates What You'll Do Assess the health status of members within the scope of licensure. Establish goals to meet identified health care needs. Plan, implement, and evaluate responses to the plan of care. Work collaboratively with the multidisciplinary team to address barriers to care. Consult with the patient's PCP, specialists, or other healthcare professionals as appropriate. Assess patient needs for community resources and make appropriate referrals. Facilitate the patient's transition within and between healthcare settings. Document accurately in the patient's electronic medical record. Provide counseling and education regarding health maintenance and disease prevention. Actively participate in organizational quality initiatives.
