You will work under the direction of a Registered Nurse or physician.Key Responsibilities:Deliver safe, effective, and compassionate nursing care to residents in accordance with individual care plans.Administer medications and treatments accurately and safelyMonitor and assess residents’ health conditions; promptly report changes to the RN or provider and family as neededDocument care, observations, and assessments in the electronic health record (EHR)Provide guidance and supervision to care providers and RA’sCommunicate effectively with physicians, families, and other care team membersParticipate in care plan meetings, quality improvement activities, and regulatory surveysUphold infection control standards and safety proceduresOther duties as assignedRequirements:High school diploma or GED requiredCompletion of an accredited practical nursing programActive, unencumbered LPN license in the state of practice; compact license may be acceptable depending on state requirementsPrior supervisory experience preferredLong-term care or similar floor experience preferredCompassionate, patient, and reliable with strong communication skillsUnderstand, respect, and maintain resident confidentialityAbility to lift, move, and assist residents safely and comfortablyMust pass required criminal background check in order to serve vulnerable adultsMust be screened TB and other state-required health care conditionsBe able to read, write, and comprehend EnglishPhysical Demands and Work Environment:Indoors, climate-controlled assisted living facility with a focus on personalized careRequires physical activity including standing, walking, lifting, and assisting residentsPossible exposure to unpleasant odors.Possible exposure to chemicals as indicated in the MSDS manual.Continuous exposure to clients and/or family members who may be under stress.This job is considered “safety-sensitive”, which is defined as “any job that includes tasks or duties that the employer reasonably believes could affect the safety and health of the employee performing the task or others”.We are an equal opportunity employer that is committed to diversity and inclusion in the workplace.
