Part-Time, Per-Visit | Growth Opportunity to Full-Time Pay: $55–$120 per visit, based on visit type Service Area: Grayson, Cooke, Fannin & Northern Collin Counties Advantx Home Care, Inc. is seeking a dependable and compassionate Registered Nurse (RN) to join our growing home health team. This position will begin part-time on a per-visit basis , with the opportunity to develop into a full-time position as your caseload grows . We are a small, locally and privately operated home health agency serving North Texas. Our clinicians have the independence and flexibility that make home health rewarding while still having access to a close-knit clinical and administrative team for support. Within the required Medicare week and patient care needs, nurses may coordinate with their patients and adjust assigned visits to the days and times that work best for both the nurse and the patient . We want our clinicians to have the independence to manage their schedules while continuing to provide consistent, reliable patient care. What You'll Do As a Home Health RN, you will provide skilled nursing services to patients in their homes throughout our North Texas service area. Responsibilities may include: Perform skilled nursing assessments and routine nursing visits Complete Start of Care (SOC) assessments Complete Resumption of Care (ROC) assessments Complete recertification and discharge assessments Complete OASIS assessments and documentation as applicable Provide wound care, medication management and patient/caregiver education Monitor patients for changes in condition and communicate concerns promptly Coordinate care with physicians, therapists, caregivers and other members of the interdisciplinary team Develop, implement and update individualized plans of care Maintain accurate and timely clinical documentation Communicate regularly with clinical leadership regarding patient needs and changes in condition Participate in care coordination and case conferencing as needed Follow agency policies and applicable Medicare, state and federal home health requirements Qualifications Current Texas Registered Nurse (RN) license in good standing Minimum of 1 year of nursing experience Home health experience strongly preferred OASIS experience preferred Strong clinical assessment and critical-thinking skills Ability to work independently in a field-based environment Strong communication, organization and time-management skills Comfortable coordinating directly with patients, caregivers, physicians and other clinicians Reliable transportation Valid driver's license and current automobile insurance Current CPR certification Our Service Area Advantx Home Care serves patients throughout: Grayson County | Cooke County | Fannin County | Northern Collin County Because this is a field-based position, applicants should be comfortable traveling to patient homes within our service area.
