Summary

The North Carolina Community Health Center Association (NCCHCA) is a membership organization for federally qualified, non-profit patient-governed health care organizations that provide care to patients regardless of insurance status or geographic isolation. NCCHCA’s mission is to promote and support patient-governed health care organizations and the populations they serve.

The Population Health Coordinator supports the planning, coordination, implementation, and evaluation of population health, care management, quality improvement, and value-based care initiatives across the Carolina Medical Home Network (CMHN) and NCCHCA.

This position serves as a key operational partner to the Chief Medical Officer and works collaboratively with CMHN leadership, care management teams, health center leaders, quality staff, payers, vendors, and external stakeholders. The Coordinator assists with project management, reporting, performance tracking, health center engagement, education initiatives, quality improvement activities, and accreditation readiness efforts.

The ideal candidate is highly organized, detail-oriented, comfortable working with healthcare data and reports, and passionate about improving health outcomes for medically underserved populations served by North Carolina’s community health centers.

Qualification Requirements:

Minimum Qualifications:

• Bachelor's degree in Public Health, Healthcare Administration, Health Informatics, Nursing, Social Work, Health Education, Business Administration, or related field required.
• Minimum of three (3) years of experience in healthcare, population health, care management, quality improvement, project coordination, community health, public health, or a related field.
• Experience working in Federally Qualified Health Centers (FQHCs), community health centers, managed care, population health, or value-based care preferred.
• Strong organizational, project management, and communication skills.
• Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, Teams, and Outlook.
• Ability to analyze reports, identify trends, and summarize findings for various audiences.
• Ability to manage multiple projects simultaneously and meet deadlines.
• Valid driver's license and ability to travel throughout North Carolina.

• Ability to maintain confidentiality and comply with HIPAA requirements.
• Ability to work independently and collaboratively in a matrixed environment.
• Experience preparing reports, presentations, meeting materials, and project documentation.
• Familiarity with NCQA, HEDIS, UDS, care management, and quality performance measures preferred.
• Experience working with electronic medical records, population health platforms, payer portals, or healthcare analytics systems preferred.

Essential Functions:

Population Health Program Support:

• Coordinate and support implementation of CMHN and NCCHCA population health initiatives.
• Assist with planning, scheduling, and monitoring population health projects and strategic initiatives.
• Maintain project plans, timelines, action-item trackers, and documentation.
• Coordinate meetings, agendas, minutes, and follow-up activities related to population health initiatives.

Reporting and Analytics Support:

• Assist in the preparation and distribution of population health, care management, quality, utilization, and performance reports.
• Support development and maintenance of dashboards, scorecards, and performance tracking tools.
• Monitor key population health metrics and identify opportunities for improvement.
• Assist with data validation and follow-up regarding data discrepancies.
• Prepare executive summaries, presentations, and reports for leadership meetings, committees, and participating health centers.

Care Management and Quality Improvement Support:

• Support care management program activities and performance monitoring.
• Assist with tracking care management outcomes, patient engagement efforts, and quality improvement initiatives.
• Coordinate Plan-Do-Study-Act (PDSA) cycles, performance improvement projects, and health center follow-up activities.
• Support implementation of evidence-based population health strategies and best practices.

Health Center Engagement and Education:

• Serve as a liaison between CMHN and participating health centers for assigned projects and initiatives.
• Coordinate educational webinars, learning collaboratives, training programs, and technical assistance activities.
• Assist with development and distribution of educational materials, toolkits, and communication resources.
• Track participation, evaluations, and outcomes related to educational activities.

NCQA and Accreditation Support:

• Assist with NCQA Care Management and Population Health Management accreditation readiness activities.
• Support policy tracking, evidence collection, documentation management, and performance monitoring.
• Maintain organized accreditation files and supporting documentation.

Health Equity and Community Health:

• Support health equity initiatives by assisting with data stratification, reporting, and performance monitoring.
• Assist in identifying disparities and opportunities to improve access, quality, and outcomes.
• Support initiatives addressing social drivers of health and community-based interventions.

Additional Responsibilities:

• Build and maintain positive relationships with internal and external stakeholders.
• Abide by department guidelines, company policies, and HIPAA regulations.
• Support special projects assigned by CMHN leadership.
• Perform other duties as assigned.

Physical Demands

While performing the duties of this job, the employee is regularly required to stand and sit; talk, hear, and use hands and fingers to handle or feel; and reach with hands and arms to operate a computer and telephone keyboard, and to reach, stoop, or kneel. Specific vision abilities required by this job include close vision requirements due to computer work. Light to moderate lifting is required, such as occasionally lifting office products and supplies up to 20 pounds. Ability to uphold the stress of traveling. Regular, predictable attendance is required, including quarter-driven hours as business demands dictate.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. The level of noise typical for the work environment of this job is moderate, less than 85 decibels (a business office with computers, printers, and traffic within the reception area). Ability to work in a confined area. Ability to sit at a computer terminal for an extended period of time. Occasional travel throughout North Carolina to visit participating health centers, or domestically for conferences or meetings, is required.
