Title
Acute Care Hospital Case Manager - Care Coordination & Discharge Planning

Schedule
Weekend rotation required; must commit to working 3 of the following holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.

Requirements
- California state license required; pending accepted only if active by start date.
- Minimum 2 years acute care hospital case management experience (care coordination and discharge planning) required.
- Travel experience required; first-time travelers may be considered (clinical screening required).
- Proficient with Microsoft applications.
- Experience with patients who have substance use, homelessness, or mental health history.
- Familiarity with advance directives/POLST and protective services processes.
- Knowledge of DRG and plan-of-care familiarity.
- Experience with InterQual criteria preferred.
- Charting system: Epic (required).

Roles & Responsibilities
- Assess patients and develop treatment/discharge plans.
- Conduct family meetings to discuss care coordination.
- Arrange for durable medical equipment (DME) and skilled nursing facility (SNF) transportation.
- Collaborate with health maintenance organizations (HMOs) and independent physician associations (IPAs).
- Ensure smooth transitions of care, including transport arrangements.

Compliance / Notes
- Orientation is approximately 40 hours.
- Pre-start modules average about 16 hours (over 20 hours requires approval).
- Return-to-office notice: 7 days maximum.
- Two references covering one year within the last three years are required.
- Floating expectation noted as No.
