Job Description:
NPORT Staffing is seeking a dedicated and compassionate Licensed Occupational Therapist (OT) to join our team and provide exceptional therapy services at one of our skilled nursing facility clients located in the beautiful Hudson Valley region of New York.

The Occupational Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to improve functional independence, mobility, activities of daily living (ADL), and overall quality of life.

The Occupational Therapist will be responsible for evaluating residents, developing individualized treatment plans, and delivering skilled occupational therapy services to help improve independence, functional abilities, and quality of life.

This is an excellent opportunity for an Occupational Therapist who is passionate about rehabilitation, enjoys working with geriatric and long-term care populations, and wants to make a meaningful impact on patient outcomes.

Responsibilities:

• Complete comprehensive occupational therapy evaluations and functional assessments
• Develop and implement individualized treatment plans based on resident goals and clinical needs.
• Provide skilled occupational therapy interventions to improve activities of daily living (ADL), mobility, safety, and independence
• Monitor the resident's progress and adjust treatment plans as needed.
• Collaborate with Physical Therapists, Speech-Language Pathologist, nursing staff, physicians, and interdisciplinary care teams.
• Educate residents, families, and caregivers regarding treatment strategies and safety recommendations
• Help residents improve independence with activities of daily activities (ADLS), including dressing, bathing, grooming, and functional mobility.
• Document evaluations, progress, notes and discharge summaries accurately and in a timely manner.
• Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries
• Follow all state, federal, and facility regulations regarding patient care documentation

Qualifications

• Current New York State Occupational Therapist License (OT)
• Graduate of an accredited Occupational Therapy Program
• Previous experience in skilled nursing, rehabilitation, long-term care, or geriatric settings preferred.
• Strong communication, organizational, and clinical skills
• Ability to work effectively as part of an interdisciplinary team
• Commitment to providing compassionate, resident-centered care
