Remote Bilingual Care Coordinator

To support its Special Needs Program, the full-time Remote Bilingual Care Coordinator (Spanish or Vietnamese) will manage outreach to members, assist with care transitions, and collaborate with RN Case Managers to address complex health needs in a remote environment. Key Responsibilities Conduct outreach to members for referrals, authorizations, and care needs Complete assessments in the Case Management system and monitor alerts for changes in member conditions Act as a liaison and advocate for members, ensuring effective communication with providers Required Qualifications Bilingual in Spanish or Vietnamese (required) Minimum 1 year of experience in a healthcare IPA setting Minimum 1 year of experience with referrals and prior authorizations High school diploma or GED (Bachelor's degree preferred or 4 additional years of experience in lieu of degree) Basic computer skills (Microsoft Outlook, Word, Excel); typing speed of 25+ WPM Virtual Vocations Inc
