The Discharge Planner plays a critical role in coordinating and facilitating the safe and efficient transition of patients from healthcare facilities to appropriate post-acute care settings. This position ensures that each patient's discharge plan is comprehensive, addressing medical, social, and emotional needs to promote optimal recovery and prevent readmissions. The Discharge Planner collaborates closely with multidisciplinary teams, including physicians, nurses, social workers, and external care providers, to develop individualized care plans that comply with regulatory requirements and payer guidelines.ResponsibilitiesMust be able to contact a patient's referral source(s) and relay necessary information to the appropriate staff members in an appropriate amount of timeMust be able to transfer requested documents to the referral source(s) and facilities relating to discharge recommendationMust be able to communicate discharge recommendations and coordinate them with the patient or appropriate staff in an appropriate amount of timeMust be able to coordinate the transfer of a patient to an outside level of care pertaining to his discharge recommendationMust be able to relay information to the appropriate Bradford Regional Offices, relating to the patient's discharge recommendations in an appropriate amount of timeMust be able to document admission information for all non-healthcare patients (admission date, referral sources, regional offices, etc)Must be able to register and order Vivitrol prescriptions for recommended patients and transfer this information to the appropriate Bradford Regional Offices or outside facilities after dischargeMust be able to use office equipment and technology (SmartCare, MicroSoft Office, email, fax machines, scanners, copiers, phones, reading medical charts, etc.) pertaining to the above tasksMust be able to work in a constant state of alertness as to perform the job safelyMust be able to work in cooperative manner with co-workers, managers, clients, and prospective clientsMust maintain a professional demeanor and appearance, must not appear impaired or disoriented, but rather must present coherent and articulate speech, kept appearance, alert, oriented, and responsive to patients' needs and concernsQualificationsBachelor's degree in social or behavioral sciences related fields or equivalent combination of education and work experienceAt least two (2) years of residential or outpatient healthcare experience requiredMust possess the ability of high level interdepartmental communication skills and knowledge of administrative workKnowledge of outside facilities and programs related to substance abuse and mental health; knowledge of healthcare marketing preferredIf recovering, must have at least 2 years of verifiable abstinenceAt Bradford Health Services, we are committed to providing exceptional care to our patients while fostering a supportive and rewarding workplace for our employees.
