While our nurses work independently, our supportive team is always here to ensure you have the resources and guidance needed to succeed. **Job Description:** We are currently seeking a skilled Per Diem Registered Nurse to conduct Uniform Assessment System (UAS) assessments throughout New York City. This role involves traveling to patients' homes to perform comprehensive assessments and create detailed care plans based on your evaluations. As an independent contractor, you will have the flexibility to manage your own schedule and choose which areas of New York City you would like to cover, with the potential to work as much as full-time if desired. If you haven’t conducted UAS assessments recently, we provide refresher training to ensure you are up-to-date with current practices and guidelines. **Key Responsibilities:** - Select and travel to patients' homes in your chosen areas across New York City to perform UAS assessments. - Evaluate and document patients' physical, emotional, and social needs. - Develop and update individualized care plans based on assessment findings. - Communicate clearly and promptly with patients and their families about care plans and available resources. - Maintain accurate and detailed documentation of all assessments. - Efficiently manage your schedule to maximize patient visits. **Requirements:** - Current and valid Registered Nurse (RN) license. - Proven experience with UAS assessments. - Excellent communication skills and the ability to manage time effectively. - Capability to work independently with minimal supervision. - Reliable transportation for patient home visits. **Desired Attributes:** - Strong organizational skills with a focus on completing tasks in a timely manner. - Compassionate, patient-centered approach to care. - Flexibility to adapt to changing schedules and priorities. **Compensation:** - $110 per patient assessment. - Opportunity to assess up to 5 patients per day. - Payments are made weekly. **Benefits:** - This is a per diem, independent contractor position with no additional benefits provided. **Company Culture:** At Allied Health Group, we foster a supportive and flexible work environment.
