UAS Assessment Nurse (RN)

Compensation

Salary Range: $90,000 - $108,000 per year + $15,000 Sign-On Bonus

Location: Various Territories, NYC
Field-Based / Onsite

Position Summary
As a UAS Assessment Nurse (RN), you will play a vital role in evaluating the clinical, psychological, and social needs of members in the community. This field-based position involves conducting in-home and nursing home assessments using the UAS-NY system, ensuring compliance with New York State Department of Health regulations. You will contribute to developing person-centered care plans and supporting improved patient outcomes.

Responsibilities

• Shift details: Monday - Friday (Business Hours)
• Hours per week: 40

? Key Duties:

• Conduct initial and reassessment visits, determining eligibility and care needs using UAS-NY
• Perform functional, psychosocial, and environmental safety assessments
• Develop and communicate individualized, person-centered service plans
• Educate members and caregivers on program benefits, services, and guidelines
• Submit accurate, timely documentation and communicate urgent needs to care teams

Requirements

? Education

• Required: Associate's Degree in Nursing
• Preferred (if applicable): Bachelor's Degree

? Licenses & Certifications

• Required: Active Registered Nurse (RN) license; UAS-NY Certification

? Skills:

• Strong knowledge of community health and managed care practices
• Excellent assessment and clinical evaluation skills
• Proficiency in Microsoft Word and Excel
• Strong communication and organizational skills
