Social Worker (LMSW/LCSW) - Case Management

Essential Functions The Social Worker LCSW provides clinical social work services for patients and their families. Perform psychosocial assessments and discharge planning for patients with complex psychosocial and medical problems. Assist patients and families in coping with hospital admission, illness, treatment, and discharge difficulties. Conduct assessment, planning, intervention, and evaluation of patient/family needs throughout hospital stay. Licenses and Certifications Licensed Clinical Social Worker (LCSW) from the Louisiana State Board of Social Work Examiners.
