Empower Lives as an Acute Care Case Manager!

Job Description:
- Start Date: ASAP
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Floating Expectation: No
- Requirements for First-Time Travelers: Must have previous travel experience
- Patient Ratios:
- Days: 1:25
- Weekends: 1:45
- Years of Experience Required: 2 years in Acute Care Case Management/Discharge Planning

Qualifications:
- Hospital Experience: Required (Behavioral health experience does not apply)
- California State License: Yes (Pending License accepted, but must be licensed upon start date)
- RTO Restrictions: No more than 7 days

Facility Highlights:
- Type of Facility: Acute Care Facility
- Scrub Color: Business Attire or Scrubs
- Daily Census: 25
- Interdisciplinary Support: Includes Physical Therapy and Social Services

Job Responsibilities:
- Assess patients and provide care coordination
- Develop treatment plans and participate in family meetings
- Knowledge of HMO and IPA systems is a plus
- Competent in arranging DME, SNF, and ambulance transport
- Ensure adherence to standards set by CMS, The Joint Commission, and more

Special Skills Required:
- Ability to manage complex patient needs
- Proficient in computerized documentation (Epic experience preferred)
- Experience with various patient demographics, including neonates, adolescents, adults, and geriatrics
- Familiarity with special procedures relevant to acute care

Additional Information:
- Weekend Coverage: Required to meet department needs (every other weekend)
- Orientation Hours: 40
- References Required: 2 references covering 1 year within the last 3 years

- Must commit to working the assigned schedule and communicate any requests upfront

- Ability to float between sister facilities may be required
