Key Responsibilities\n

• Ensure patient safety by implementing clinical best practices and crisis intervention strategies when necessary.

• Provide ongoing case management support, collaborating with the Utilization Review Department and other stakeholders regarding authorization and payment processes.

• Initiate comprehensive discharge planning as soon as a client is admitted, ensuring that a seamless transition begins from day one.

• Lead discharge sessions to confirm that clients understand their coping strategies, follow-up plans, and available resources post-treatment.

• Confirm discharge appointments with clients and their families to facilitate a smooth transition to follow-up care.
\n
• Continuously communicate with referral sources and outpatient providers throughout a client's care journey to maintain continuity and support.

Compensation and Benefits\n

We offer a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience and caseload.
