Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify clients' emotional, behavioral, and social needs, establishing individualized treatment plans aimed at promoting patient recovery.

• Develop and implement discharge plans in collaboration with multidisciplinary healthcare professionals, ensuring continuity of care upon client discharge from treatment.

• Engage in counseling and provide crisis intervention for clients, utilizing established social work interventions to address immediate concerns related to mental health.

• Serve as a liaison by interacting with physicians and other healthcare providers, effectively communicating updates regarding patient progress, needs, and concerns.

Compensation and Benefits\n
