Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments of patients to identify and address barriers to treatment and enhance overall health status.

• Collaborate with the Social Work Manager to develop effective treatment plans based on the unique needs of each patient, ensuring compliance with the NASW Code of Ethics.

• Provide outpatient mental health therapy using evidence-based modalities, including both individual and group formats, tailored to the specific challenges faced by patients.

• Engage in regular documentation of social work activities and patient progress in electronic medical records to facilitate comprehensive care delivery.

• Educate patients and families about social work resources and medical recommendations to support recovery and promote optimum psychosocial functioning.

• Advocate for patients' needs within the community, establishing connections to resources that enhance social support and address environmental factors impacting health.

• Maintain awareness of community resources and documentation standards to ensure efficient coordination of care and effective reporting of clinical outcomes.

Patient Population / Client Focus\n

This role focuses on providing care primarily to patients with complex psychosocial challenges, including individuals experiencing terminal illnesses and their families. You will work alongside patients who may have co-existing mental health concerns, offering them tailored support during a critical time in their lives. Your interventions will aim to improve their quality of life and facilitate their ability to navigate emotional and practical challenges.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution and a current LCSW credential are required.

• Between six months to one year of clinical experience within health or mental health settings is necessary, focusing on the specific populations designated by the organization.

Compensation and Benefits\n

The salary for this position ranges from USD 85,000 to 100,000 annually, commensurate with experience and qualifications. We also offer a sign-on bonus to enhance your onboarding experience.
