This role will be conducted remotely, allowing for flexibility and adaptability in your work routine. Daily interactions will involve virtual meetings and case discussions where your input will be pivotal for developing effective care strategies.

Key Responsibilities\n

• Conduct thorough assessments of clients’ psychosocial histories, identifying areas for intervention and support.

• Collaborate with the practitioner team to identify and de-escalate potential risks, ensuring timely response and adherence to safety protocols.

• Lead crisis interventions when necessary, including high-risk situations that require immediate escalation to emergency services or mandatory reporting.

• Keep Clinical Leads informed of newly identified safeguarding risks, working proactively to develop mitigation plans.
\n
• Monitor outcomes through rigorous feedback collection and a focus on evidence-based practices to enhance the quality of services provided.

• Participate in ongoing training efforts to remain current with safeguarding policies and ensure that they are integrated into day-to-day activities.

Patient Population / Client Focus\n

The social worker will engage with a diverse population including individuals and families facing end-of-life issues, bereavement, and various mental health challenges related to substance use, trauma, anxiety, and depression. You will be instrumental in helping clients navigate their emotional health and providing support tailored to their unique perspectives and situations.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program is essential for this position.

• Candidates must possess an active Licensed Clinical Social Worker (LCSW) designation.
