Job Description:

Seeking an experienced RN or Social Worker for a role requiring prior CA Dignity experience. Candidates must participate in care coordination and demonstrate proficiency in executing psychiatric holds under 5150 certification.

Responsibilities:

• Conducting patient assessments and care coordination.
• Utilizing the Cerner charting system for documentation.

Requirements:

• Must have prior experience with CA Dignity.
• LCSW and BLS certifications are required.
• Must be 5150 certified in California.
• Requires two supervisory references, one within the last 12 months and another within three years.
• Not employed directly by any facility associated with CommonSpirit within the past year, except as PRN through an outside firm.
