Pharmacist - Outpatient Oncology Infusion - Per Diem

What's this role all about? As a per diem Outpatient Pharmacist, you’ll be an integral part of the patient care team, working closely with medical, nursing, and pharmacy staff to provide outstanding care to cancer patients. Responsibilities Provide comprehensive care for outpatient oncology patients across various cancer‑related disease states. Review and verify chemotherapy orders to ensure accuracy and safety. Educate patients on their treatments and care plans. Monitor therapeutic drugs to optimize therapy and improve outcomes. Participate in oncology disease‑related groups to support guideline, SOP development, updates, and new drug monographs. Provide accurate drug information to healthcare professionals and patients. Qualifications Educational Requirements: Pharm.D. or an equivalent degree from an accredited U.S. pharmacy school. Completion of a PGY1 and/or PGY2 residency or equivalent postgraduate experience. License and Certification: Current pharmacist licensure in the State of California. BLS certification as per Pharmacy Policy and Procedure. Ability to comprehend patient medical management plans. Ability to assess and apply current pharmacological and biopharmaceutical principles for drug selection and use in a clinical setting. Expertise in extracting pertinent information from patients or other sources and linking it to the care plan. Strong verbal and written communication skills. Ability to teach and serve as a preceptor for training programs. Capability to prepare drugs needed for immediate administration. Skill in providing recommendations and monitoring drug use. Knowledge of laws, regulations, and standards related to pharmacy practice. Adaptability to change and ability to prioritize workflow. Evaluation of medication regimens to meet therapeutic needs. Learning aptitude for automated information systems. Competency in pharmaceutical calculations, compounding, and pharmacokinetics. Patient‑centered communication skills. Computer proficiency necessary to perform job functions. Exceptions approved by the Chief Pharmacy Officer or designee.
