Outpatient Infusion Pharmacist - PD at Kaiser Permanente Panorama City, CA

Job Summary The Outpatient Infusion Pharmacist provides comprehensive clinical and distributive pharmaceutical services under indirect or direct supervision. Responsibilities include evaluating patient variables, developing medication plans, performing clinical assessments and monitoring, reconciling medications, dispensing and compounding products, and educating patients and caregivers. The pharmacist collaborates with medical staff, manages medication therapy, and performs other duties as required. Essential Responsibilities Upholds Kaiser Permanente Policies, Procedures, and applicable regulations. Performs sterile and non‑sterile compounding, pharmaceutical calculations, and age/disease specific pharmacokinetic dosing. Reviews and monitors IV medication therapy for patients in home care, long‑term care, hospice, and other outpatient settings. Communicates and collaborates with patients, caregivers, and healthcare team to optimize patient care. Reviews all medication orders for appropriateness based on data interpretation and patient outcomes, clarifying orders and intervening when necessary. Follows approved protocols to manage medication therapy, including dose adjustments and laboratory test orders. Interviews patients for drug use, allergies, and other pertinent clinical information. Documents all information in medical records and computer systems per regulations. Advises patients and families on medication directions, storage, coordination with diet, adherence, precautions, and device usage. Guides safe disposal of medications and proper IV line care. Participates in quality assurance, medication safety, and error prevention activities. Functions independently in day‑to‑day clinical and administrative duties. Participates in problem identification and resolution. Demonstrates competency through annual outpatient infusion pharmacy competency exams. Develops and tests protocols, computer programs, and pharmacokinetic tools. Participates in assigned training and develops educational programs for pharmacy staff and patients. Participates in committees, utilization review, and quality management programs. Supports a collaborative Labor‑Management Partnership environment. Performs other duties as required. Basic Qualifications Experience: Minimum of 1500 hours of acute care or infusion pharmacy experience within the last three (3) years, or successful completion of an ASHP accredited clinical pharmacy practice residency within the last five (5) years. Education: Degree in Pharmacy meeting minimum licensure education requirements. Licenses and Registrations: California Pharmacist License, Controlled Substance Utilization Review and Evaluation System Registration, National Provider Identifier, and Basic Life Support within three (3) months of hire. Additional Requirements: Demonstrated knowledge of patient variables, comprehensive pharmacological knowledge, aseptic technique, communication skills, regulatory knowledge, information extraction, pharmacy computer systems, and acute care medication knowledge. Preferred Qualifications Experience collecting patient‑specific information for infusion services suitability. Experience determining route, access, and device suitability for infusion therapy. Experience establishing and evaluating monitoring parameters for safety and efficacy. Experience managing infusion therapy (antibiotics, parenteral nutrition, PCA pain management). Strong interdisciplinary teamwork, communication, and educational skills. ASHP accredited clinical pharmacy practice residency and Doctorate of Pharmacy (Pharm.D.).
