In this primarily remote role, you will engage with clients through virtual sessions, utilizing our secure telehealth platform to provide comprehensive support. • Conduct comprehensive psychosocial assessments to gather vital information about clients’ mental health challenges as well as their strengths and available resources.

• Provide individual, family, and group therapy employing evidence-based treatment modalities tailored to the unique circumstances of each client.
\n
• Develop individualized treatment plans with clear goals and interventions, regularly reviewing and adjusting these plans based on client progress.
\n
• Collaborate with medical professionals to ensure all aspects of care are coordinated, focusing on holistic treatment approaches that integrate physical and mental health.

• Facilitate crisis interventions, perform risk assessments, and develop safety plans to safeguard the well-being of clients in acute distress.
\n
• Document all client interactions and treatment plans accurately and timely, ensuring compliance with legal standards and ethical practices.
\n
• Lead therapeutic group sessions, psychoeducation classes, and support groups, fostering community among clients and facilitating shared experiences.

Your primary focus will be on individuals facing emotional and psychological challenges related to serious illnesses, including grief and end-of-life issues. You will engage with a diverse clientele of all ages, implementing personalized care strategies that honor their unique life experiences, cultural backgrounds, and emotional needs. Your role is crucial in promoting resilience and ensuring that each client feels supported and understood throughout their treatment journey.

• A Master’s degree in Social Work from an accredited institution.

• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated understanding of evidence-based therapeutic practices and modalities.

• Excellent interpersonal and communication skills to foster meaningful client relationships and collaborate effectively with a multidisciplinary team.

• A minimum of two years of experience working in a mental health or outpatient care setting, particularly with a focus on hospice or palliative care.

• Proficient knowledge of trauma-informed care principles and the ability to apply them in therapeutic practice.

• Experience with crisis intervention and high-risk assessments, including safety planning.
