Work Environment

This full-time position requires working onsite at our rehabilitation center, where you will be immersed in a collaborative atmosphere. Our facility is designed to provide a calm and supportive environment for recovery, featuring modern treatment rooms and rehabilitation equipment. The schedule is Monday to Friday, from 9:00 AM to 5:00 PM, allowing for consistent routines that benefit both staff and patients.

Key Responsibilities

• Evaluate patients upon admission, conducting thorough assessments to determine their rehabilitation needs, physical limitations, and goals.
• Develop and implement individualized treatment plans utilizing evidence-based interventions such as therapeutic exercises, manual therapy, and modalities tailored to each patient's specific needs.
• Monitor and document patients' progress regularly, adjusting treatment plans based on ongoing evaluations to ensure optimal outcomes.
• Educate patients and their families on exercises and strategies to promote recovery and prevent further injury, enhancing their understanding of their own health.
• Collaborate closely with physicians, nursing staff, and other healthcare professionals to ensure an integrated approach to patient care.
• Lead therapy sessions for an average of seven patients per day, providing direct care and support as they work towards their recovery goals.
• Stay current with industry trends and advances in rehabilitation therapy by participating in professional development opportunities and continuous education.

Patient Population / Client Focus

Your work will primarily focus on a diverse patient population in a skilled nursing facility setting, including individuals recovering from surgeries, strokes, falls, and other conditions affecting mobility and function. Your empathetic approach and clinical expertise will directly contribute to improving their physical capabilities and overall well-being. By addressing the unique challenges each patient faces, you will help them reclaim their independence and functionality.

Qualifications

• Valid New York State Physical Therapist License is required.
• A Bachelor's degree in Physical Therapy is mandatory; a Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) is preferred.
• A minimum of two years of experience in a similar clinical setting, particularly in skilled nursing or rehabilitation.

Preferred Experience

• Experience in implementing rehabilitation programs for geriatric populations is highly desired.
• Familiarity with using electronic health record (EHR) systems for documentation and treatment management.
• Proficiency in various therapeutic techniques relevant to rehabilitation therapy, including but not limited to, therapeutic exercise, pain management strategies, and functional mobility training.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience and qualifications.
