As a Physical Therapist at Moving & Grooving Rehab Center, you will work in a structured yet flexible home care setting. Key Responsibilities

• Provide individualized therapy to an average of seven patients daily, utilizing evidence-based practices to enhance mobility and rehabilitation.
• Conduct thorough assessments to design and implement personalized therapy plans that reflect each patient's specific goals and needs.
• Guide patients in performing mobility and strengthening exercises, ensuring correct technique to promote safety and effectiveness.
• Communicate clearly and compassionately with patients and their families, offering education and emotional support throughout the recovery process.
• Maintain accurate and detailed patient records, tracking progress and making necessary adjustments to therapy plans.
• Collaborate with a multidisciplinary care team to coordinate comprehensive patient care, sharing insights and data to optimize treatment outcomes.

Patient Population / Client Focus

Your role will involve working primarily with adult patients who may be recovering from orthopedic surgeries, neurological disorders, or other acute and chronic conditions. You will have the opportunity to build meaningful relationships with patients, understanding their challenges, and celebrating their successes as they progress through their rehabilitation journey.

Qualifications

• Current New York State Physical Therapist License.
• Bachelor's Degree in Physical Therapy; a Master's or Doctorate in Physical Therapy is preferred.
• At least two years of hands-on experience in a clinical or home care setting, demonstrating success in patient rehabilitation.

Preferred Experience

• Experience using electronic health record (EHR) systems for documenting patient care and outcomes.
• Background in geriatric therapy or experience with diverse patient populations is a plus.
