Position Overview

We are seeking a dedicated and skilled Physical Therapist to join our team at Echo Rehabilitation Center, located in Montgomery, New York. In this full-time, onsite role, you will be instrumental in providing high-quality physical therapy services to patients in a home care setting. Work Environment

Your work will primarily take place in patients' homes, allowing for personalized and flexible therapy sessions tailored to each individual's specific environment. This setting enables you to observe the patients in their daily surroundings and create functional goals that reflect their lifestyle. You'll collaborate closely with other healthcare providers to ensure a holistic approach to each patient's treatment.

Key Responsibilities

• Conduct thorough evaluations of new patients, including assessments of physical capabilities and limitations.
• Develop and implement individualized treatment plans that incorporate evidence-based practices to enhance mobility and function.
• Provide skilled physical therapy treatments, including therapeutic exercise, manual therapy techniques, and patient education on home exercises.
• Monitor and document patient progress, adjusting treatment plans based on the latest assessments and outcomes.
• Educate patients and their families about their conditions, treatment processes, and home care strategies, ensuring clear communication and support.
• Manage daily caseload efficiently, typically seeing seven patients each day, while ensuring all documentation and administrative tasks are completed in a timely manner.

Patient Population / Client Focus

You will primarily work with adults and seniors recovering from a variety of conditions, including but not limited to post-operative rehabilitation, stroke recovery, orthopaedic injuries, and chronic pain management. Your role will be crucial in helping these individuals regain independence and improve their overall mobility in a familiar and comfortable setting.

Qualifications

• Active New York State Physical Therapist License.
• Bachelor's Degree in Physical Therapy; a Master's (MSPT) or Doctorate (DPT) is preferred.
• Two years or more of direct experience in physical therapy, particularly in outpatient or home care settings.

Preferred Experience

• Experience with geriatric populations and familiarity with common age-related conditions.
• Background in manual therapy techniques and patient education strategies.
• Proficiency in using rehabilitation tools and technology to improve treatment outcomes.

Compensation and Benefits

This role offers a competitive annual salary ranging from $100,000 to $120,000 , commensurate with experience and qualifications. In addition to a generous pay structure, we provide a comprehensive benefits package which includes:
