Key Responsibilities

• Evaluate and assess patients' physical conditions through comprehensive evaluations, determining the best course of treatment based on individual needs.
• Administer, modify, and supervise treatment plans for a diverse patient base, ensuring targeted interventions that facilitate optimal recovery.
• Manage a caseload of up to 7 patients each day, providing individualized attention and adjustments to treatment protocols as necessary.
• Document patient progress through electronic health record systems, meeting compliance standards for patient care and treatment documentation.
• Educate patients and their families on treatment methods and home exercise programs, ensuring understanding and promoting adherence to therapy activities.
• Collaborate with physicians, nurses, and rehabilitation specialists to devise specialized care plans that reflect a comprehensive understanding of each patient's needs.
• Assist patients with mobility issues and functional tasks using therapeutic equipment and techniques, fostering independence where possible.

Patient Population / Client Focus

The Physical Therapist will work primarily with elderly patients, including those recovering from surgery, injury, or chronic conditions such as arthritis and stroke. Our patient population is diverse, requiring tailored approaches to meet varying physical capabilities and rehabilitation goals. Understanding the unique needs of each patient is essential in our multidisciplinary approach to rehabilitation.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's Degree in Physical Therapy is mandatory, with a Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) preferred.
• Strong communication and interpersonal skills to effectively engage with patients and their families.

Preferred Experience

• Two or more years of experience in a clinical physical therapy setting, ideally within a skilled nursing or rehabilitation facility.
• Experience working with geriatric populations and patients recovering from orthopedic surgeries is highly desirable.
• Familiarity with electronic health record (EHR) systems to maintain accurate patient documentation.

Compensation and Benefits

The selected candidate will receive a competitive annual salary ranging from $100,000 to $120,000 , commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package that includes:
