Cityblock Health is seeking an Ambulatory Care Clinical Pharmacist to deliver Comprehensive Medication Management (CMM) in collaboration with our hub-based and virtual clinical teams. You will support disease management, optimize regimens, and participate in multidisciplinary rounds to improve patient outcomes. The role requires PGY1 and PGY-2 residencies or equivalent ambulatory care experience, with a focus on safe, rational pharmacotherapy and patient-centered care. #J-18808-Ljbffr Cityblock Health, Inc.
