Licensed Practical Nurse

As the Licensed Practical Nurse (LPN), you will be responsible for providing direct resident care and supervision of residents’ needs based on their individualized service/care plan. You will monitor residents for changes in condition and signs/symptoms of illness, in accordance with federal and state laws and regulations, as well as the policies of the Community. In partnership with the Health Services Director, you will communicate and document changes or concerns regarding residents and assist in supervising the care team. Responsibilities Provide direct resident care and supervise residents’ needs based on individualized service/care plans Monitor residents for changes in condition and signs/symptoms of illness Communicate and document changes or concerns regarding residents in partnership with the Health Services Director Assist in supervising the care team and ensuring compliance with policies Qualifications A valid Licensed Practical Nurse (LPN) license; a Compact license or the ability to obtain one is encouraged. An unencumbered LPN license is required. Minimum of at least one year of successful experience as an LPN in assisted living or long-term care Experience administering medication and treatment plans per doctor's orders and community policies Knowledge of state regulations pertaining to resident care and understanding the importance of being state survey ready, every day Experience as a mandatory reporter and ability to intervene, report, and investigate all suspected or alleged abuse or theft from a resident Current driver’s license Benefits/Why Join Us At Trustwell, we believe in creating a work environment that fosters growth and satisfaction.
