Remote Community Health Worker: Outreach & Education

MERCY HOME HEALTH

Mercy Home Health is seeking a Community Health Worker for a remote role supporting grant-funded work in Putnam and Van Wert counties, Ohio. The role focuses on outreach, education, and facilitating access to essential services, serving as a liaison between community members and health and social service systems.

The CHW will build relationships, identify barriers to care, and connect individuals to resources.
