This position offers the flexibility of a remote work setting that focuses on school facilities, allowing for a balance of in-home visits and coordination with community resources. As a full-time employee, you will generally work Monday through Friday, from 8:00 AM to 4:30 PM, with the potential for occasional evening or weekend shifts to meet client needs. Key Responsibilities\n

In this role, you will:

• Conduct thorough assessments of seniors' psychosocial, emotional, and social needs, identifying factors that influence their well-being.

• Collaborate closely with clients, families, and other stakeholders to develop personalized care plans that align with individual goals and preferences.

• Provide counseling and emotional support, helping seniors cope with life transitions, challenges, and health-related issues.

• Advocate for clients’ rights, ensuring they have access to services and resources that meet their needs.

Patient Population / Client Focus\n

Your primary focus will be on the senior population, addressing their unique psychological, emotional, and social needs. Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Minimum of two years of professional experience in social work, ideally within a community center or geriatric-focused environment.

Preferred Experience\n

The position offers a competitive annual salary range of $60,000 to $100,000, based on experience and qualifications.
