As a Licensed Clinical Social Worker at Gotham Enterprises Ltd, your day-to-day responsibilities will encompass the following:

• Conduct comprehensive psychosocial assessments via structured interviews and thorough reviews of medical records to identify emotional, social, and environmental factors influencing patient recovery.

• Provide empathetic counseling and emotional support to patients and their families, effectively addressing adjustment issues that arise during the rehabilitation process.

• Collaborate with fellow rehabilitation professionals to create detailed, individualized treatment plans that incorporate the unique psychosocial needs of each patient.

• Assist patients in navigating community resources, including connecting them with support groups, vocational rehabilitation services, and housing assistance as required.

• Educate patients and their families on available community and post-rehabilitation resources to ensure a smooth transition to life outside of our center.

• Participate actively in discharge planning, ensuring that patients transition safely back home or to other care settings with a thorough understanding of their continued needs.

To thrive in this role, candidates must possess the following:

• A Master’s degree in Social Work from an accredited institution.
