Responsibilities Evaluate and treat residents with various physical therapy needs, including assessing range of motion, strength, flexibility, and balance Develop and implement individualized treatment plans, incorporating modalities, exercises, and manual therapy techniques Collaborate with our multidisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care Conduct resident assessments and develop discharge plans, ensuring seamless transitions Maintain accurate and detailed records of patient progress and treatment plans Participate in staff meetings, in-service training, and continuing education to stay current with best practices and industry advancements Requirements A Doctor of Physical Therapy (DPT) degree or equivalent Licensure to practice as a Physical Therapist in the state of New York Part-time schedule (exact hours to be discussed during the interview process) About Us Windsor Park Nursing Home is a family-oriented facility that provides compassionate care in a comfortable and supportive environment.
