Job Details RN-Registered Nurse // Case Management Onsite Contract Starts 7/20/2026 – 12 weeks Day Shift 8 hours/day 5 days/week Travel Information TRAVEL – RN – CASE MANAGER – COST CENTER (917) Facility Overview HCA Florida Lake Monroe is an accredited 221‑bed acute care hospital located in Sanford, Florida, serving Seminole and West Volusia counties. The hospital offers a wide range of services including a full‑service cardiovascular program, Level II trauma services, advanced stroke care, orthopedic and spine care, advanced robotic surgery, inpatient rehabilitation, and 24/7 emergency care. Unit Details Hospital: Central Florida Regional Hospital Location: Sanford, FL Address: 1401 W Seminole Blvd, Sanford, FL 32771 Unit: 3East and 3 West Unit Size: 42–50 beds Patient Ratios: 22 Floor Plan: Computer‑based documentation (Meditech, Midas) Work Hours: 8:00 a.m.–4:30 p.m., with weekend rotation Floating Policy: No Responsibilities Acute Care Case Management in UR, DCP and Transitions of Care – Home Health and Skilled Nursing Facility set ups. Rounding with interdisciplinary teams (MDs and IDT) Qualifications RN license Experience with Meditech and Midas Advanced DCP Assessment Skills, InterQual experience Certification in Care Management (preferred) Unit Guidelines & Policies Unit staffing includes secretaries and tech support availability.
