Physical Therapist

Seeking a Full-Time Physical Therapist to provide personalized home health care. Responsibilities include delivering skilled physical therapy based on individualized treatment plans, evaluating patient needs, setting rehabilitation goals, and adjusting treatments for optimal outcomes. The role involves collaborating with an interdisciplinary team, educating patients and caregivers on exercises and safety, documenting progress accurately, and supporting patient independence and quality of life at home. Qualifications Master’s degree in Physical Therapy from an accredited program. Current PT license in the state of employment or eligibility to obtain. CPR certification required. Strong communication, assessment, and problem-solving skills. Ability to work independently in diverse home environments. Valid driver’s license, auto insurance, and reliable transportation. Preferred Experience: In-home therapy, geriatric, orthopedic, or neurologic rehab, and experience within a mobile or interdisciplinary team setting.
