QMHP / Community Support Specialist
Chicago | Contracted 
As a QMHP/Community Support Specialist, you'll work directly with clients in their homes and communities,
helping them build the skills and confidence to manage symptoms and live more independently. This is handson, relationship-driven work that makes a visible difference.
What You'll Do
● Deliver individualized community support services tied to each client's treatment plan.
● Teach coping, communication, problem-solving, and daily-living skills.
● Provide psychoeducation on mental health, wellness, and relapse prevention.
● Help clients build crisis-prevention and wellness strategies.
● Support community integration and connection to natural and formal supports.
What You'll Bring
● Meets the Medicaid definition of a QMHP.
● Relevant behavioral health experience.
● Strong written, verbal, and interpersonal skills.
● Valid driver's license, reliable transportation, and auto insurance for community travel.
Nice to Have
● Master's degree in counseling, social work, psychology, or a related field.
● Experience with community-based behavioral health services.
