WPHPA - Practice Registered Nurse

Job Summary The Registered Nurse (RN) provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, planning and intervening in actual or potential health problems, including evaluation of the patient's response to the care. These responsibilities also include patient/family/significant other education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals. Essential Functions Understands and adheres to the WPH Performance Standards, Policies and Behaviors. Demonstrates the ability to perform clinical, technical, service and administrative tasks. Systematically and continuously assesses patient physical, psychosocial and spiritual health status and knowledge base to determine patient’s health care and educational needs. Provides and/or delegates patient care according to the care goals, sets priorities based on patient need, and triages all sick and walk‑in patients. Establishes therapeutic and supportive relationships with patients and family, acting as a resource person. Coordinates client care including referral back to primary provider, assesses needs and initiates referrals (VNS, Social Work). Handles telephone triage and prescription requests according to procedure. Communicates normal and abnormal test results in collaboration with provider including appropriate follow‑up. Performs therapeutic treatments and procedures, assists with procedures and administers oral/IM/IV/SQ/Inhaled medications. Coordinates delegates and/or provides all other patient care activities. Coordinates in‑patient admissions and ER referrals, acting as a patient advocate. Documents according to standards of care, promotes and provides for patient safety in accordance with state and federal guidelines. Participates in performance improvement activities, provides presentations to care team members and ensures all staff participates as a member of the patient care team. Attends in‑service and continuing education programs, demonstrates a professional, courteous and respectful attitude in dealing with patients, families and significant others. Informs and educates patient/family to enable participation in their care and decision‑making. Evaluates the effectiveness of interventions and patient responses and outcomes to care, collaborating with team members to make necessary changes to patient plan of care. Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers. Performs all other related duties as assigned. Qualifications BSN required or MSN. 1‑3years prior nursing experience in a health care setting preferred. Knowledge of current nursing, medical and health trends and practices in patient care (medium proficiency). White Plains Hospital Mandate: Registered Nurse. NewYork State Boards must hold a valid RN license. Certifications Must hold BLS Basic Life Support – American Heart Association upon hire. Must hold ACLS Advanced Cardiac Life Support – American Heart Association upon hire for all RNs working in the Cancer Center or Center for Advanced Medicine & Surgery (CAMS).
